What is the Education Benefits Form?

The Education Benefits Form is a household form for families to complete. The form is used to determine how much funding our district will receive for education programs from the state and federal government: 

Purpose: The form determines a student or household's eligibility for funding. The number of eligible students determines how much funding the school receives. 

Benefits: Funding can help schools provide additional support and services for students, such as: 

  • Instructional staff
  • Teaching supplies and materials 
  • Counselors and social workers 
  • School nurses 
  • Professional learning for staff 
  • Parent and community engagement supplies and activities 
  • Technology funding
  • College application fees and Scholarship applications
  • AP Testing
  • HAYA Scholarship applications
  • Discounted cell phone, cable, and internet fees for families
  • Grocery benefits through EBT

Who completes the form: Parents and guardians complete and sign the form. 

What to include: The form includes information about the household, such as:

  • Student information, including name, grade level, school and homeless, migrant, runaway, or foster child status
  • Benefits received, such as Food Assistance Program (FAP), Family Independence Program (FIP), or Food Distribution Program on Indian Reservations (FDPIR) 
  • Household size and income ranges
